To speed up the registration
process, you can find all the forms, documents and other items you will be
needing for your Hajj or Umrah experience right here on this page. Follow
the checklist below to make sure you've got everything ready to be
FedExd to our offices. No other means of
mailing are accepted.
To reserve your spot, we will need the following immediately:

Copy of the passport

$2,000 deposit per person by check, money
order or cashiers check (NO CREDIT CARD).
FORMS & APPLICATIONS

 Visa
Application
The Saudi Consulate will need a Visa
Application for either your Hajj or Umrah trip.

 Barakah Hajj & Umrah Registration Form
We require you to fill out your information and sign to our terms and
agreement before traveling with us. You may fill out this form by either
downloading it (click the above link) and sending it back to us (via fax,
scan & e-mail, or mail); or online.
LETTERS & CERTIFICATES
-
Certificate of Vaccinations
List of vaccinations and other pertinent information can be found
here.

Copy of Marriage Certificate
This certificate is necessary for the traveling married couple and
MUST be notarized.

Copy of Birth Certificate for each child traveling
Make sure to get a notarized copy for each child
accompanying you.

Mahram Support Letter
All ladies above the age of 45 traveling alone will need to obtain a
letter of support from either a husband, brother or father simply stating
that permission has been granted for the lady in question to go for Hajj.
Ensure that the lady signs and her mahram signs. If

Proof of Conversion to Islam/Status in Islam
Muslims with "non-Islamic names" must obtain written proof of their
Islamic faith from the director of their mosque or Islamic center.
CONCERNING YOUR PASSPORT

Valid passport for at least six (6) months beyond travel date.
Check your expiration date now.

Five (5) passport-sized photos with white background. Full names and
passport number/country must be written on the back of each picture in
pencil.

US re-entry permit/travel document or copy of Green Card for
non-US citizens.
FEES & PAYMENTS

Send one (1) CASHIER'S CHECK for $274.40 USD per each
person.
Make them each payable to The Unified Agent Office (Remember,
that's one check per
person).
NO MONEY ORDERS OR PERSONAL CHECKS WILL BE ACCEPTED!
Note that your name and passport number must be written on each
cashiers check in memo.

Barakah Hajj & Umrah Fees
Please send the fees for the package you have selected. You may write
a personal check (cashier's check not necessary), made payable to
Barakah Hajj.
MISCELLANEOUS

Digital Photo for Hajj Participants
One digital picture of each person traveling must be e-mailed to
barakahhajj@gmail.com no later
than November 5, 2007. It is recommended that this photo be one of your
passport photos (if it is not electronic to begin with, consider scanning
it). Photo should be at least 300 DPI and will be used for your Barakah
Hajj & Umrah ID card. Any questions regarding the digital photo should be
directed to barakahhajj@gmail.com.
Once all of your documents are complete and ready to go, you may mail them
via FedEx (in a prepaid return FedEx
